How to Register
If you do not want to attend the practice and complete the forms you may download and print off a registration form and a new patient questionnaire, fill it out and post it to the practice. Please note there are separate new patient questionnaires for adults and children and a different registration form to register a new born baby.
Medical Treatment
Medical treatment is available from the date of registration but if you are on regular medication please ensure you have 2 weeks worth of medication from your previous GP Surgery. Please contact reception for further information.
Health Check
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
